2013 Fall Family Camp
Bert Adams Scout Reservation

Read all of the information below.
Register your family by the end of the day on Oct 19. If you did not register at the Oct 12th Pack Meeting, then register by sending an e-mail to Alan Seal with the following info about your family's attendees:
Name of your Cub Scout
Names of any participating siblings
Names of any non-participating siblings (no charge)
Names of adults (no charge)
Number of Pack Meal plans that will be purchased
Turn in a Medical Form for EVERY attendee from your family at the Oct 12th Pack Meeting or at the FFC Attendee Meeting on Oct 18th. Health forms must include copy of front and back of insurance card. If you cannot attend, then please contact Alan for alternate submission method.
Submit payment by Oct 19th in person or online. Make checks payable to Pack 815.
Who: All Cub Scout Ranks and their families
Pack 815 Registration deadline: Oct 19th
FFC 2013 Theme: “Chickenapolis 500”
Registration Cost: $10 per scout and $10 per participating sibling (i.e., sibling who will participate in the FFC activty rotations). There is no charge for adults or for non-participating siblings.
Pack Meal Plan: The Pack Grubmasters will be preparing meals for the Pack on Saturday and Sunday. Meal plans are $10 per person and include breakfast, lunch, and dinner on Saturday, and breakfast on Sunday.

Required Health Forms: This is an overnight trip of less than 72 hours, so by BSA rules, a separate Annual BSA Health and Medical Record Form, Parts A & B, must be submitted for each person attending (scouts, parents, siblings, etc.). This form is valid for one year, so submitting it now will cover you for every event until this time next year. The form is attached below. Please download the form and populate as much as possible before printing it out. Bring completed forms to the Pack Meeting on Oct 12th, or contact Greg Foreman for other delivery options.
Youth Protection Training: All Adult Leaders should complete Youth Protection Training before attending this trip. Also, it is highly recommended that all parents also complete this training. It is available on-line and can be completed in less than an hour. Go to www.myscouting.org, create a log-in account, and take the course. Once you've completed the course, save or print a copy of your completion certificate and submit it to Greg Foreman.
Parking is enforced. No parking in camp by order of the Rangers. You must use the designated areas. If the parking lot is distant from your campsite, use a shuttle service. You may drive to your campsite to unload, but then move your car before setting up camp.
